Why do I need to fill out demographics every year?
We used to just ask patients every visit if there were any changes to their insurance, address, phone number, etc.
Most of the time people will say that nothing has changed.
However, we often get denials for payments due to changes in insurance companies. We also have trouble reaching patients with results sometimes only to find that the phone number has actually changed. Other times we will have correspondence or bills sent back to us due to address changes.
Therefore, to be sure we have the most up-to-date information we need to have patients fill out the demographic forms once a year.
